YOU WILL NEED:

·         Registration packet filled out in full (access to the folder here) or can be picked up at any of our schools

·         Proof of age (birth certificate)

·         Current immunization records

·         Name and address of previously enrolled school with most recent report card / school records (copy of IEP or 504 if applicable)

·         Four (4) proofs of residency

       (Examples: utility bills (2), driver’s license, current pay stub with address, deed or lease, tax statements, voter registration, court order, motor vehicle registration)

·      Notarized residency affidavit
